Program Application

The Long-Term Care Plus (LTC+) team needed a fillable and Letter sized application for health teams to use to apply to the program. Teams needed to be able to save their progress and resume work on the application after closing the file. The filled information needed to be printable.

CFHI was going through an amalgamation with another Pan-Canadian Health Organization. The branding used was to be minimal and not related to either organization.
